Nursing degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

A rewarding opportunity is available for a Registered Nurse with expertise in rehabilitation and long-term care. This role focuses on providing comprehensive care to patients in nursing home and assisted living settings, helping to manage their health effectively and avoid unnecessary hospitalizations. The position supports insurance members by delivering monthly health assessments and coordinating care to promote successful transitions back to home environments.

Schedule: • Monday through Friday, 8 AM to 5 PM

• No weekends or holidays

Key Responsibilities: • Collaborate closely with Nurse Practitioners (NPs) to support member care and coordinate services

• Conduct routine monthly visits and review resident status to track progress toward health goals

• Participate in meetings representing the NP and communicate updates to the interdisciplinary care team

• Facilitate care coordination within skilled nursing facilities (SNFs) to meet resident needs

• Address clinical and quality improvement measures, including identifying and closing gaps in care

• Engage regularly with residents, families, facility staff, and care team members

• Assist with resident rounds and support advanced care planning

• Travel between locations may be required for acute visits, typically focusing efforts at one site per day

Qualifications: • ASN or BSN degree

• Current New Hampshire RN license

• Basic Life Support (BLS) certification

• Minimum of 1 year experience in long-term care, care coordination, or care management preferred
